Register with our practice

The doctors welcome new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire.

Alternatively please ask at reception after 10.30am if you wish to complete paper registration forms .

You have the right to see any of the doctors, but that doctor may not necessarily be available on the day of your choice.

You can only make an appointment when your  registration has been completed by the practice.  For both online and paper applications the registration process takes between three and five working days from submission.

We register new patients  who live within our practice boundary  which includes the London Borough of Richmond north of the River Thames and a small part of London Borough of Hounslow close to the surgery.

Our practice boundary was revised several years ago and we continue to welcome and register:

• Newborns or adopted babies of parents who live outside the current boundary but who were registered at our practice before the boundary change
• First-degree relatives of patients already registered at our practice, who are living at the same home address

Additionally, if you are an existing patient and move to a new address that falls outside our our current practice boundary, you will unfortunately need to register elsewhere.
