NON-NHS SERVICES – PRIVATE FEES

The following services are not covered by the NHS and therefore attract a fee.

Many letters, certificates, reports and forms requested by patients, employers, insurance companies and other organisations fall outside the NHS services provided by the practice.

Fees reflect the GP’s time required to review the medical record, consider the request and complete the relevant documentation.

All non-NHS work is undertaken at the discretion of the GP. The GP may decline a request if they do not have sufficient information or do not feel able to provide the opinion or certification requested.

NHS clinical work takes priority over private administrative work.

 

SIMPLE LETTERS, CERTIFICATES & FORMS

Straightforward certificate or statement of fact:   £30

For brief factual confirmation based on information already contained in the medical record.

Examples include:

  • confirmation of a known medical condition
  • confirmation of treatment
  • confirmation of medication
  • confirmation of relevant medical history
  • simple “To Whom It May Concern” letter
  • brief factual letter to an employer, school, university or other organisation

Simple form requiring GP completion/signature:   £30

Applies where only limited factual information is required and no detailed review of the medical record is necessary.

 

EMPLOYMENT, SICKNESS & OCCUPATIONAL FORMS

Private sick note / incapacity certificate for an employer:   £40

Applies where a certificate is requested which is not an NHS statutory fit note.

Bespoke employer letter regarding health or workplace circumstances:   £40

For requests requiring information beyond a straightforward statement of fact or that normally provided on an NHS fit note.

Employer medical questionnaire or occupational health form:   £50

For a short questionnaire or form requiring review of the medical record but no examination.

Employment medical requiring examination and completion of a form:   From £100

Some occupational health assessments may be more appropriately undertaken by an occupational health physician, in which case the practice may decline the request.

 

INSURANCE FORMS & REPORTS

Short insurance certificate or form:   £40

Examples include:

  • private medical insurance pre-treatment or pre-authorisation forms
  • accident insurance claim forms
  • sickness or income protection certificates
  • holiday cancellation insurance certificates
  • travel insurance medical certificates
  • certificates confirming inability to travel

Targeted / detailed insurance report:   From £100

For reports requiring review and interpretation of the medical record relating to a particular condition or limited area of medical history, including where a GP opinion is requested.

General Practitioner Report (GPR):   From £150

For more comprehensive reports, including those requested for:

  • life assurance
  • income protection
  • critical illness insurance
  • other insurance underwriting purposes

Additional information following completion of an insurance report:   From £30

Complex insurance report requiring extensive review of the medical record:   Fee to be agreed in advance

The final fee will depend on the length and complexity of the report and the extent of medical record review required.

 

TRAVEL & AVIATION

Airline special assistance / medical assistance form:   £40

For completion of airline forms relating to medical or mobility assistance.

Letter confirming the need to carry medication or medical equipment on an aircraft:   £30

Examples include:

  • injectable medication
  • needles or syringes
  • insulin
  • liquid medication
  • CPAP equipment
  • other prescribed medical equipment

Other straightforward factual travel letter:   £30

For example, confirmation of an existing medical condition for travel purposes.

Travel insurance certificates and cancellation forms are charged under Insurance Forms & Reports above.

The practice may decline forms requiring specialist investigations, certification or examinations which the GP is not authorised or appropriately qualified to provide.

 

SCHOOL, COLLEGE & UNIVERSITY

Confirmation of illness or absence affecting school attendance:   £30

For brief factual confirmation requested by a parent, school or other educational establishment.

Mitigating or extenuating circumstances letter:   £40

For school, college or university requests requiring information regarding illness or other medical circumstances affecting attendance, examinations or academic performance.

University or college medical form:   £50

For forms requiring review of the medical record but no medical examination.

University or college medical form requiring examination:   £100

 

EXERCISE, SPORT & PHYSICAL ACTIVITY

Health club / gym medical certificate:   £40

Sports or competition medical / participation form:   From £50

Where review of the medical record is required but no examination is necessary.

Sports or competition medical requiring examination:   From £100

Expedition or adventure activity medical requiring examination:   From £100

Certain specialist activities may require assessment by an appropriately accredited doctor and may therefore not be offered by the practice.

 

DRIVING & LICENSING MEDICALS

HGV / LGV / PSV / PCV medical (D4):   £100

Taxi, private hire, TfL / PCO or local authority driver medical:   £100

Other occupational driving medical:   From £100

Where eyesight testing is required, patients may be asked to have the relevant assessment performed by an optician before attending.

Where the DVLA commissions a medical report directly, the fee is payable in accordance with the relevant DVLA arrangements rather than under this patient fee schedule.

 

PROFESSIONAL, OVERSEAS & OTHER OFFICIAL FORMS

Basic medical declaration or official form:   From £40

For straightforward confirmation based on information contained within the medical record.

Professional registration medical form:   From £50

Overseas employment, study or camp medical form:   From £50

Where review of the medical record is required but no examination is necessary.

Overseas employment, study or camp medical requiring examination:   From £100

Other official form requiring review of the medical record but no examination:   From £50

Other official form requiring a medical examination:   From £100

 

PASSPORT APPLICATIONS

Passport countersignatures are not normally provided as a GP service.

Doctors can only act as passport referees or countersignatories in circumstances permitted by HM Passport Office, including where the doctor knows the applicant personally rather than solely through the doctor-patient relationship.

 

PRIVATE PRESCRIPTIONS

Private travel prescription where permitted under NHS regulations:   £20 per prescription

For example, malaria prophylaxis or medication prescribed in anticipation of an illness arising while abroad, where clinically appropriate.

The fee relates to issuing the private prescription. The cost of the medication is payable separately to the dispensing pharmacy.

 

SERVICES FOR WHICH PATIENTS ARE NOT NORMALLY CHARGED

Certain certificates and services are governed by NHS contractual or statutory requirements and do not fall within the private fee schedule.

These include, where applicable:

  • NHS sick notes / Med 3 certificates
  • statutory certificates required by the Department for Work and Pensions
  • certification for exemption or discount from Council Tax due to severe mental impairment
  • certification required to establish unfitness for jury service
  • notification of statutory infectious diseases
  • other certificates which legislation or the NHS contract requires the doctor to provide without charging the patient

Where a third-party organisation commissions a report directly, any appropriate fee may be payable by that organisation rather than by the patient.

 

IMPORTANT INFORMATION

Fees shown are the practice’s usual charges. Where a fee is shown as “From“, the final charge will depend on the length and complexity of the form or report and the amount of medical record review required.

Where a higher fee is likely to apply, the patient or requesting organisation will be advised before the work is undertaken.

Completion of private work is not part of the NHS service and is undertaken at the GP’s discretion.

A GP cannot certify information or provide an opinion that they cannot reasonably verify from the medical record, consultation or examination.

The practice may decline requests:

  • requiring specialist expertise
  • asking the GP to guarantee future fitness
  • requiring inappropriate retrospective certification
  • requiring tests or examinations which the practice does not provide
  • where insufficient information is available to provide the requested opinion or certification
